The official responsible for D.C.’s 911 call center resigned Tuesday amid complaints about poor performance within the agency.

Jennifer Greene was appointed director of D.C.’s Office of Unified Communication (OUC) in 2011 by former D.C. Mayor Vincent Gray, and since she took over the 911 call center, a man died across the street from a fire station when fire fighters were sent to the wrong address, the Washington Times reported.

In January of 2014, Medric Mills experienced a heart attack, from which he later died, near a fire station in Washington, D.C. He was shopping with his daughter when he suddenly collapsed. Mills’s daughter ran to the fire station across the street, but instead of helping her, firefighters in the station reportedly ignored her.

A firefighter at the front desk announced twice on the station’s PA system that a medical emergency was happening outside and called for the lieutenant, but the lieutenant didn’t respond to either request, according to the Washington City Paper.

Eventually emergency personell responding to a 911 call about the incident arrived, but only after they went to the wrong address. In total, it took 23 minutes from the time of the 911 call before Mills was in an ambulance.

In January of this year, the agency came under fire again after a Metro train stopped in a tunnell filled with smoke, leaving many passengers sick and one woman dead.

The Washington Post reported that fire fighters sent to free the passengers from the stuck train had to use their cell phones to communicate with each other because their radios wouldn’t work in the tunnel. The new digital radios, administered by the OUC, were reportedly to blame.

Greene has been working with the D.C. police force for nearly 30 years, but will now be replaced at the OUC by D.C. Homeland Security and Emergency Management Agency Director Chris Geldart while they search for her replacement.
