Ganna Shelekh is a 19-year-old Olympian from Ukraine and a world-class, athletically superior pole vaulter.
But in one of her three attempts, Shelekh never made it off the ground to clear the 14 foot 4 inch height.
The epic fail cost her the chance to qualify for the finals and the opportunity to win gold.
Americans Jennifer Suhr and Becky Holliday and the two-time defending Russian gold medalist Elena Isinbaeva all vaulted more than 14 foot 9 inches, sending them to the final competition.
